Can a visual inspection replace the spark test?

No. A pinhole can be invisible to the eye yet become a rust site in the acid vapour space within a season. The 1500 V holiday-spark test is what finds discontinuities across the whole surface; a visual check misses the very defects that matter most in sewage service.

Is 1500 V enough, or should it be higher?

1500 V is the qualified test for the standard fused enamel grade; thicker or special grades may use a different specified voltage. The point is that the voltage is tied to the coating grade and applied to the full surface, not that a single high number is always better.

Does verification matter more for some stages than others?

It matters everywhere, but most acutely for the vapour space of sludge holding and digestion, where acid forms, and for any tank with grit or abrasive solids. Equalisation and aeration also need verified coatings because they see the widest chemical envelope.
